The scheduled treatment (planned treatment)

If you are travelling to us for planned medical treatment, you must

obtain authorisation for the assumption of costs from your home health insurance fund before your departure.

For this purpose, you will receive form S2 (formerly E112) from your home health insurance fund.

 

Only with this document can you prove your entitlement to planned medical treatment in Germany.

You have to present the S2 form to the German statutory health insurance fund, you have chosen to help you out, and you will receive a national proof of entitlement from them.

 

You must present this national proof of entitlement to treatment to the clinic on the day of admission. Only with this proof is your treatment then possible under the same care and payment conditions as for German nationals. The only presentation of your electronic health card (health insurance card) is not sufficient.

If you are unable to present us with a national proof of entitlement from a German assisting statutory health insurance, admission can only be carried out as a self-pay patient.

A subsequent conversion or reimbursement of costs by the German assisting health insurance fund is unfortunately not possible.

 

This applies to both inpatient hospital treatment and outpatient visits.
