Patient information
You were transferred to our Interdisciplinary Stroke Center for further care. To facilitate your visit, you are kindly requested to bring along your full set of documents including the following:
- insurance ID card, referral from your family doctor or specialist
- medical reports from previous doctors’ visits and hospitalizations. If you do not have them, please ask your doctor for these documents.
- any existing images from previous examinations, such as from computer tomography (CT), angiography or magnetic resonance imaging (MRI)
- a listing of the medication you are currently taking
