(The form can also be saved on your PC for later use)

All clients are kindly requested to bring along their vaccination cards, if available. Otherwise, new cards will be issued upon consultation.

Please direct yourself to the waiting room first, and fill out the provided registration form in legible print letters. As above mentioned, you can prepare the registration already online (see above "Fill in and Print out"). You will be called personally by the doctor.

Travel vaccinations are covered by some health insurance companies (please contact your insurer directly).

Payment of services has to be settled directly after each consultation, according to the pricelist posted in the waiting area (cash or bank card).

For minors, consent by parent or legal guardian is required.
